Skilled Nursing
Medication compliance
If you or your loved one needs help understanding a new health condition, how or when to take medications, or the importance of managing high blood pressure Patriot nurses are there to answer any questions you may have.
IV Therapy
Patriot nurses will educate and train you and your caregivers to ensure you are confident in managing your IV medications at home.
Skilled Wound Care
Patriot nurses are trained to assess, evaluate, and identify a variety of wounds including surgical wounds, diabetic wounds, and pressure ulcers. Our nurses will create a personalized wound care plan and educate the patient and caregivers on how to care for these wounds, as well as supervise the healing of the wound.
Wound Vac Care
Wound vacuums can seem intimidating, but Patriot nurses are here to help. Our nurses are trained to supervise and operate wound vacs, as well as teach patients and caregivers so they are comfortable maintaining them in the home.
Therapy Services
Physical Therapy
Patriot physical therapists evaluate a patient’s unique needs and create a personalized treatment plan based on their evaluation. Treatment plans can include safe walking and bed transfer training, fall prevention, and pain management.
Occupational Therapy
Patriot occupational therapists work with patients to improve their activities of daily living. These activities include bathing, dressing, eating, and strength training.
Speech Therapy
Patriot speech therapists work with patients to help improve a patient’s ability to safely speak, swallow, or communicate.
Other In-Home Services
Skilled Home Health Aid
Patriot provides skilled home health aids that assist patients in bathing.
Medical Social Work
Medical social workers work with patients and their families to educate them on community resources and support systems that may be available to them.
Telehealth services
Telehealth – or virtual visits – allows our nurses, therapists, and nurse practitioners to safely and securely discuss your progress, answer your questions, and provide guidance regarding your condition all during a video call.
