RN Admission Nurse

As a RN Admission Nurse, you will be doing the start of care evaluation, doing a complete home assessment, and creating the patient’s plan of care. For our RN Admission Nurse’s, Patriot Homecare has developed an innovative admission process that minimizes the OASIS and admission documentation requirements of the RN.

As a RN Admission Nurse, you will:

  • Have coordination of total patient care
  • Evaluate the effectiveness of nursing service to the patient and family
  • Prepare and present patient’s record to the Clinical Record Review Committee as indicated
  • Participate in the patient’s plan of care during the admission process
  • All other duties as assigned

Qualifications:

  • Must be a graduate from an accredited School of Nursing
  • Must be licensed in the State as a Registered Nurse
  • One or more years of experience in community/home health agency or hospital preferred
  • Must have knowledge of Medicare and Medicaid guidelines
  • Must have a criminal background check
  • Must have a current BLS CPR certification

Job Conditions:

  • Must have a driver’s license and be willing and able to drive to patient’s residences
  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform and demonstrate patient care
  • Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients

RN Case Manager

As a Registered Nurse Case Manager, you will provide nursing care in accordance with the patient’s plan of care to include comprehensive health and psychologic evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities, and direct nursing care. We also strive to make sure your family time comes first. As a RN Case Manager, there is no Weekend On Call. Patriot Homecare has developed an innovative admission process that minimizes the OASIS and admission documentation requirements of the RN.

As a Registered Nurse Case Manager, you will:

  • Have coordination of total patient care
  • Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis

  • Submit clinical no less often than weekly and progress notes and other clinical record forms outlining the services rendered as indicated

As an Occupation Therapist, you will provide skilled occupational therapy service on an intermittent basis to patients in their homes in accordance with the physician’s orders.

As an Occupational Therapist, you will:

  • Assist the physician in evaluating the patient’s level of function by applying diagnostic and prognostic procedures
  • Conduct initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments
  • Assure that the physician’s orders are appropriate and discusses necessary changes
  • Teach, supervise, and counsel the family and patient in the total Occupational Therapy program and other related problems of the patient at home
  • Communicate with the physician on a regular basis regarding patient’s status, obtaining additional orders, if necessary

  • The ability to drive and ability to access patients’ homes which may not be routinely wheelchair accessible are required

This position will build relationships in Pittsburgh Hospitals and other potential home care referral sources who have patients discharging to the agency coverage area including Trumbull, Mahoning, Columbiana, Stark, Portage and Summit Counties and have experience in management of online referrals through Allscripts and other systems as well as in-person account management.

Established relationships with UPMC, VA, Allegheny General Preferred.

As a Hospital Liaison, you will:

  • Analyze the potential of the company’s service area to determine target markets
  • Visit Doctor’s offices, hospital discharge desks, assisted living facilities, and other possible sources of referrals to present Agency credentials and obtain patient referrals
  • Analyze the company’s organization to determine its strengths and weaknesses
  • Analyze past and current marketing data
  • Complete an analysis of the company’s “product” line
  • Analyze patient/company relationships
  • Develop sales/marketing objectives and sales projections
  • Develop a marketing plan, identifying priorities and sets a reasonable timetable
  • Implement marketing plan staying within established timetable
  • Review and evaluate the analyses and plan on an established basis

Qualifications:

  • At least 1 year of sales experience preferred
  • At least one year experience in marketing, nursing, or social work interacting with health agencies/professionals
  • Must be organized and detailed oriented
  • Must be able to communicate effectively in writing, orally in English
  • Must be able to interact with health professionals at all levels
  • Must have criminal background check

  • This position is primarily outside sales which essentially involves visiting referral sources daily, driving from place to place as well as telephone communication with patients and accounts
  • This position requires minimal lifting of office records and printouts
  • For this position, you must have the ability to read 12 point and larger type, communicate clearly, and be able to hear adequately on the telephone with no more than an amplifier
  • Must be able to use a PC, calculator, multi-line telephone, and other office equipment
