Licensed Practical Nurse – (LPN)

Akron, OH

Full-Time

The LPN is to provide skilled nursing care under the direct supervision of the RN Case Manager.

Responsibilities:

  • Assist the RN in carrying out the plan of care
  • Assist the RN in performing specialized procedures as directed b the RN under physician’s orders
  • Assist with the preparation, implementation, and continuing evaluation of the patient care plan
  • Help teach the patient appropiate self-care techniques
  • Prepare clinical progress notes
  • Assist with the rehabilitation of patients according to the medical to the medical care plan

Job Conditions:

  • The ability to communicate well, both verbally and in writing, in English is required
  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform patent care
  • Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and the turning of patients

Qualifications:

  • Graduation from an accredited School of Practical Nursing
  • Must be licensed in the State of Ohio as a LPN
  • One or more years in home health experience is preferred
  • Must have a criminal background check
  • Must have current CPR certification

RN Case Manager

Akron, OH

Full-Time

To provide nursing care in accordance with the patient’s plan of care to include comprehensive health and psychology evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities, and direct nursing care. As a RN Case Manager, there is no Weekend On Call and minimal Oasis requirements.

Responsibilities:

  • Coordination of total patient care
  • Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis
  • Prepare and present patient’s record to the Clinical Record Review Committee as indicated
  • Submit clinical no less often than weekly and progress notes and other clinical record forms outlining the services rendered as indicated
  • Participate in the patient’s discharge planning process
  • All other duties as assigned

  • Must have a driver’s license and be willing and able to drive to patient’s residences
  • The ability to access patients’ homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient’s condition and to perform and demonstrate patient care
  • Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients

  • One or more years of experience in community/home health agency or hospital preferred

RN Admission Nurse

Full-Time

The Admissions Nurse is responsible for assuring programs are effectively represented to patients and families through pre-assessments, information sharing, and/or admissions in a patients home. This position works with the intake team, referring physician and other referral sources.

All admission paperwork will be completed by a scribe assistant. This position is Saturday and Sunday, 12 hour work shift.
